PLAINTIFF CONSENT FORM

Pursuant to the Fair Labor Standards Act, 29 U.S.C. § 216(b) (the "Act"), I hereby consent to become a party plaintiff in a collective action lawsuit filed in the United States District Court for the Western District of Missouri in Kansas City against the City of Kansas City, Missouri (“City”), alleging violations of the Act based on the City’s failure to properly calculate and pay overtime wages. These violations include, but are not limited to, the City’s failure to count all hours worked during holidays, mandatory overtime, hold-over shifts, and call-back assignments toward the applicable overtime thresholds under the Act.

I designate the law firm of Boyd Kenter Thomas & Parrish LLC of Independence, Missouri, to represent me in this action and to make decisions on my behalf concerning the litigation, including any settlement discussions and agreements.

I declare under penalty of perjury that the foregoing is true and correct.
